WASHINGTON – Don’t be offended if your doctor writes that you’re SOB, or that an exam detected BS.
The aim is to help, not insult: A project is beginning to test if patients fare better when given fast electronic access to more of their medical chart — the detailed notes that doctors record about you during and after every visit. You just might have to look up some of the technical jargon, like those abbreviations for “shortness of breath” and “bowel sounds.”
Didn’t know about those notes? Researchers involved in the “OpenNotes” project say they are surprised at how many patients don’t.
“You really have to be a partner with your doctor to do well,” says Dr. Tom Delbanco of Harvard and Beth Israel Deaconess Medical Center, who heads the study and thinks better use of those notes will help.
“It’s your body. It’s your record. It’s your illness. You should have ready access to everything about it.”
Yes, your clinic may have an electronic records system that lets you log in to make an appointment, check your cholesterol test or review your medications. But Delbanco and nursing colleague Jan Walker have found few include those doctor notes that provide details about a patient’s health.

Review: ‘Cookbook Collector’ overcooks good story (AP) – NIMBRUNG.NET
Emily is a 28-year-old millionaire, thanks to the dot-com boom. She works as CEO of a Web startup. Jess is a graduate student at Berkeley, where she is studying philosophy.
Jess works in a store that sells rare books. In her spare time, she hands out leaflets about saving trees. Emily wants Jess to get a job with health insurance. “What if something happens to you?” she asks. “What if, for example, you fall out of a redwood?”
“The Cookbook Collector” also follows a number of supporting characters, including George, who owns the bookstore where Jess works, and Jonathan, Emily’s long-distance boyfriend.
George collects old books, and doesn’t care too much or need the money he makes from selling them. On the surface, he’s annoyed by Jess’ tree-hugging ways. Yet he cannot stop thinking about her when she’s not around.
Jonathan works for his own Internet company in Cambridge, Mass. He and Emily are experiencing the excitement of the Internet explosion. Emily naively shares a trade secret with him that sets off warning bells early in the story.
“The Cookbook Collector” would be a better read with fewer supporting characters. Random characters and subplots about religion and politics dull the story.
The best parts of the book trace Jess and Emily’s relationship with their significant others over time.
It’s also interesting to read about the excitement of the dot-com boom of the late 1990s, when profitless Internet startups wooed investors. We all know how it ended, but it seemed fun while it lasted.

Struggling states seeking more aid from Washington (AP) – NIMBRUNG.NET
BOSTON – Governors hamstrung by the sluggish economic rebound in their states and bound to balance their own budgets are pressing anew for Washington to step up with more help, some say even if it means adding to the nation’s red ink.
Republicans and Democrats alike wrestled with how to capitalize on a fledgling rebound as they talked dollars and sense at their summer meeting just days into a new budget year and as the economy shapes dozens of gubernatorial races across the country.
“All states still are facing tough fiscal situations even though I do believe we’re in recovery,” said West Virginia Gov. Joe Manchin, a Democrat who’s taking over as chairman of the National Governors Association.
Added Gov. Jim Douglas, R-Vt., the outgoing chairman: “Governors have done what is necessary to get through this” — repeatedly cutting budgets, restructuring government, laying off workers and draining rainy day funds.
But both men said states can’t continue to climb out of the recession alone, and the NGA renewed its bipartisan appeal for Congress to pass stalled jobs legislation that includes billions of dollars in aid to states.

ER doctors: Lawsuit fears lead to overtesting (AP) – NIMBRUNG.NET
CHICAGO – Fast decisions on life-and-death cases are the bread and butter of hospital emergency rooms. Nowhere do doctors face greater pressures to overtest and overtreat.
The fear of missing something weighs heavily on every doctor’s mind. But the stakes are highest in the ER, and that fear often leads to extra blood tests and imaging scans for what may be harmless chest pains, run-of-the-mill head bumps, and non-threatening stomachaches.
Many ER doctors say the No. 1 reason is fear of malpractice lawsuits. “It has everything to do with it,” said Dr. Angela Gardner, president of the American College of Emergency Physicians.
The fast ER pace plays a role, too: It’s much quicker to order a test than to ask a patient lots of questions to make sure that test is really needed.
“It takes time to explain pros and cons. Doctors like to check a box that orders a CT scan and go on to the next patient,” said Dr. Jeffrey Kline, an emergency physician at Carolinas Medical Center in Charlotte, N.C.

ATLANTA – How much money would it take to get you to lose some serious weight? $100? $500?
Many employers are betting they can find your price. At least a third of U.S. companies offer financial incentives, or are planning to introduce them, to get their employees to lose weight or get healthier in other ways.
“There’s been an explosion of interest in this,” said Dr. Kevin Volpp, director of the University of Pennsylvania’s Center for Health Incentives.
Take OhioHealth, a hospital chain whose workforce is mostly overweight. The company last year embarked on a program that paid employees to wear pedometers and get paid for walking. The more they walk, the more they win — up to $500 a year.
Anecdotal success stories are everywhere. Half of the 9,000 employees at the chain’s five main hospitals signed up, more than $377,000 in rewards have already been paid out, and many workers tell of weight loss and a sudden need for slimmer clothes.
But does will this kind of effort really put a permanent dent in American’s seemingly intractable obesity problem? Not likely.
